About Yasmin

"I have worked as a senior rehabilitation case manager since 2007, providing rehabilitation support for clients with both severe and complex disabilities (including spinal cord injury, severe burns, permanent blindness and/or amputations), specialising in clients with both primary and secondary mental health conditions and behavioural challenges. I have worked with clients ranging from young adults to ageing clients (over 65).
I provide case/injury management using a person-centred approach through:

Initial Needs Assessments:

• identifying clients’ strengths, barriers and need for specific services
• identifying need for additional assessments i.e. Vocational Assessment and testing, Transferrable Skills Analysis, Labour Market Analysis, Workplace assessments, vocational/adjustment to disability counselling, employment assessments
• identifying required support services, making recommendations.
Planning and implementation:
• engagement and coordination with providers to drive client’s goals and overcome barriers
• completing additional assessments e.g. Vocational Assessment
• applying strategies to promote participation and self-management
• monitor, review and revise service provision and supports, client goals, rehabilitation plans and strategies to ensure continuity and ongoing satisfaction.

I believe case management must be client focused, considering the person’s medical, psychosocial and vocational needs. Actively working with my clients delivers the best outcomes; even in resistant situations, collaboration is the key to positive change.

My clients are largely complex, requiring the skills and life experience to support them face the barriers encountered with significant injuries/illnesses and secondary psychological injuries. Every person is different; their rehabilitation will depend on the nature and severity of their injury/illness and their individual circumstances, requiring a holistic/multi-disciplinary approach. I have collaborated extensively with other allied health professionals and believe communication and transparency with all parties is essential for success. My experience with a diverse range of clients has developed my skills to organically understand and provide my clients with additional supports as required whether it be language, cultural or learning challenges."
