My Plan

If you're a Lifetime Care or Workers Care Program participant we can help you work out an ongoing plan for your treatment, rehabilitation and care needs.

Through the Lifetime Care and Workers Care Program, we pay for your ongoing treatment, rehabilitation and care needs related to your motor accident or workplace injuries.

Once you have been through the process of being assessed and accepted into one of our schemes, a planning facilitator to work with you to develop a plan to identify what you want to do in the future, and how we can help get you there.

We aim to put you at the centre of the planning process, enabling you to decide what kinds of things you want to do and achieve in your life.

My Plan with icare Lifetime Care and the icare Workers Care Programs

If you’re an icare Lifetime Care participant or a worker in the icare Workers Care Program, a planning facilitator, who is usually your case manager or coordinator, will work with you to get to know you and help you to identify your goals and support needs using a framework called ‘My Plan’.

Preparing your plan involves meeting with your planning facilitator, as well as talking to the people who support you.

In the first stage after your injury, the focus will usually be on recovery and rehabilitation. Some of the goals in your plan will reflect your continuing recovery.

As you progress, treatment and therapy may continue, but you’ll also begin to plan your activities and participation in life at home, work and in the community.

Your planning facilitator will meet with you regularly and help you to review your plan, and identify any new goals and support you need.

They’ll help you request funding for the treatment, rehabilitation and care support required because of your injury.

Staying involved in planning for your treatment, rehabilitation and care needs following a severe injury helps keep the focus on you and your situation – what’s important to you and what you want to do.
