About Geraldine
"I completed a graduate diploma in rehabilitation counselling at Sydney University in 2000 following an undergraduate Bachelor of Arts degree, majoring in psychology and education. I then worked in brain injury rehabilitation case management, in both metropolitan and regional settings. My membership of the Australian Society of Rehabilitation Counsellors (ASORC) has been ongoing since 2004.
My expertise is in assisting people with brain, spinal cord and severe physical injury improve their independence in everyday life through well-coordinated, person-centred support services, including case management. I have extensive experience working as part of multidisciplinary teams to support people find their way in the rehabilitation process. Of particular interest is discharge planning, transitional life points (for example, moving from school to work), and the early stages of rehabilitation.
I love to bring a sense of order to the sometimes overwhelming experience of recovery from serious injury. I am an approved icare case manager with experience in comprehensive third party and workers compensation."

Moore Rehab Outcomes provides evidence-based rehabilitation, medicolegal and case management services for CTP Care, Lifetime Care, and Workers Care clients. We provide services to regional NSW (Victoria to Queensland borders), plus Sydney and ACT regions.
Our team of over 20 experienced clinicians of occupational therapists, case managers, rehabilitation counsellors and social workers are trained to achieve better health outcomes. We are able to assist managing urgent matters or requests if your case manager is on leave, and we can reallocate files as necessary.
Our clinicians are carefully selected based on experience / skills and are able to work collaboratively with the participant, their treating team and family to help make the rehabilitation journey as smooth and supported as possible.
