"I have been an Approved Case Manager with Lifetime Care since 2015 and have extensive case management experience working with adults with traumatic brain injury and spinal cord injury. I have in depth knowledge and experience of the Lifetime Care scheme to support participants and their families with planning and co-ordinating treatment, rehabilitation and care services.
I have experience with working with a multidisciplinary team to effectively facilitate an individual’s engagement in their rehabilitation and treatment, motivating and supporting them to achieve optimum health outcomes. As a Registered Nurse I have extensive clinical experience working in areas of neurology, surgical, orthopaedic, high dependency care and aged care in both the private and public sectors. In addition, I have over 20 years of experience working as a Rehabilitation Consultant specialising in the provision of case management services.
I am highly skilled in performing all areas of case management, care coordination, rehabilitation assessment, worksite assessments and liaison with all stakeholders. I specialise in providing proactive, collaborative and people centered case management services for participants. I am empathetic and dedicated in assisting individuals and their families with all aspects of their rehabilitation following an injury."
