Transition home
If our relationship with you and your family begins in hospital or a rehabilitation centre, a care manager will visit before discharge, ensuring a smooth transition from hospital to home.

We understand how important it is to be comfortable with those in your support team, so we encourage you and your family to meet members of our team in advance.
